This is our intake form. Please fill out as completely as you can, print, and sign.

Medical History Form

This worksheet will give us better information on your skin's "settings," especially for laser and facial procedures. Please circle whichever applies to you, and add the total score at the bottom.

Before LASER HAIR REMOVAL:

For Laser Hair Removal, make sure you shave the areaTHE NIGHT BEFORE before your appointment. We also offer numbing cream you can purchase prior to your session. When applied 1 HOUR BEFORE treatment, the numbing cream can significantly reduce potential irritation from the laser itself, which is medical-grade and therefore quite powerful.

Before a CHEMICAL PEEL, MICRODERMABRASION, IPL, or LASER SKIN TIGHTENING:

Make sure you have removed your makeup prior to your session, as cosmetic products will interfere with these procedures. After your treatment, you will be given a sunscreen moisturizer to protect your face from UV radiation, as the skin may feel "raw" once exfoliation occurs. You should also not plan on re-applying your makeup after your session for at least 24 hours, as the skin may be sensitive even to minor irritants and toners.
